The Department of Labor (DOL) has released new FMLA certification forms that provide employees with the required notices in a new electronic fillable pdf. The new forms will help reduce contact and have also been simplified, making them easier to complete by all parties involved. It is important to note that the DOL does not require the use of any specific form and you may continue using older versions of the forms if you prefer.
